Serum Carbohydrate Antigen 549 (CA-549) has been identified as a marker for breast cancer. The purpose of this study is to assay the clinical and diagnostic significance of serum (CA-549) at different stages of endometriosis. Methods: A serum of (120) women patients with endometriosis. That classified according to the guidance of the “American Society of Reproductive Medicine” (ASRM) guidelines is staged (I, II, III, and IV). And (50) healthy women as the control group. Data were analyzed by statistical software (SPSS. 2017. Statistical Package for the Social Sciences) Statistical Version (25.0). IBM SPSS Statistics. , which involved mean, (ROC), Pearson correlation regression, and a P value regarded significant if (≤ 0.05). Results: The mean of (CA-549) in the control group (12.58) is statistically higher than the mean in stages (Ⅰ, Ⅱ, Ⅲ, and Ⅳ) which equal (4.70, 3.90, 2.82, and 1.88). Statistically difference for the mean of (E2) in stage Ⅰ was (410.33), stage Ⅱ (449.40), stage Ⅲ (551.00), stage Ⅳ (605.16), vs control (332.50). (ROC) test was used; (CA-549) marker had the highest AUC (0.99) with (95%) sensitivity and (100%) specificity, and (E2) that had AUC of (0.93) with (100%) sensitivity and (99.82%) specificity. (E2) had a statistical significant negative moderate correlation with CA-549. Conclusion: This is the first study assessing the carbohydrate antigen-549 in women with different stages of endometriosis. Serum (CA -549) was a negative moderate correlation with the estradiol (E2) class and might be a potential marker for predicting the diagnosis and progression of this condition.
